Your Rights
Get a Copy of Your Health Record
You may ask to see or receive an electronic or paper copy of health information we maintain about you. We will provide a copy or summary as required by law, generally within the time allowed by HIPAA. A reasonable, cost-based fee may apply when permitted.
Ask Us to Correct Your Record
You may ask us to correct health information you believe is incorrect or incomplete. We may deny the request in some circumstances, but we will explain the reason in writing when required.

You may ask us to contact you in a particular way or at a particular location. We will accommodate reasonable requests.
Ask Us to Limit What We Use or Share
You may ask us not to use or share certain health information for treatment, payment, or health care operations. We are not always required to agree to these requests.
If you pay for a service in full out of pocket and ask us not to disclose information about that service to a health plan for payment or health care operations, we will honor the request when required by law.

If you have given someone medical power of attorney, or another person is legally authorized to act as your personal representative, that person may exercise your rights as permitted by law.
File a Complaint
You may contact The Massage Therapist if you believe your privacy rights have been violated. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights.
Your Choices
For certain health information, you may tell us your preferences about what we share. When applicable, this may include information shared with family members, friends, or others involved in your care or payment for your care.
If you are unable to tell us your preference, we may share information when permitted by law and when we determine that doing so is in your best interest.
We will obtain your written authorization before using or disclosing your health information for purposes that require authorization under HIPAA. You may revoke an authorization in writing as permitted by law, except to the extent that we have already acted in reliance on it.
