Appointments & Scheduling
Deposits
A deposit equal to half of the scheduled session price is required when booking private-pay appointments. The remaining balance is due at the time of service.
Insurance-based appointments may follow different payment procedures depending on the client’s plan and verified benefits.
Cancellations & Missed Appointments
Appointments require at least 24 hours’ notice for cancellation or rescheduling. Cancellations made with less than 24 hours’ notice and missed appointments may result in forfeiture of the appointment deposit.
When an appointment is being redeemed through a session package or membership, a late cancellation or missed appointment may result in that session being deducted from the remaining sessions.
Insurance cannot be billed for an appointment that did not occur. Any applicable cancellation or missed-appointment fee is the client’s responsibility and will not be submitted to insurance.
Late Arrivals
Sessions are scheduled for a specific amount of time. Arriving late may shorten the available treatment time so that appointments following yours can begin as scheduled. The full scheduled appointment fee may still apply.
Packages, Memberships & Gift Certificates
Session Packages
Three-session packages expire 6 months from the date of purchase. Six-session packages expire 12 months from the date of purchase.
Session packages are non-refundable and non-transferable. Late cancellations and missed appointments are deducted from the remaining package sessions in accordance with the cancellation policy.
Memberships
Membership sessions and benefits are subject to the terms provided at enrollment. Additional sessions purchased at member pricing are for the active member and are subject to appointment availability.
Gift Certificates
Gift certificates may be applied toward services offered by The Massage Therapist. Terms applicable to a gift certificate are provided at the time of purchase.
Professional Boundaries & Treatment
Professional Conduct
Clients may request changes to pressure, positioning, techniques, draping, or areas being treated at any time. A client may also ask that a technique be discontinued or end the session at any time.
Health Information
Clients are responsible for providing accurate health information and notifying the therapist of relevant changes in health, medications, pregnancy status, injuries, surgeries, or other conditions that may affect treatment.
Massage therapy is not a substitute for medical diagnosis or medical treatment.
Payment & Financial Responsibility
Payment
Payment is due at the time of service unless other arrangements apply through an insurance plan or another approved program. Accepted payment methods are available during booking and at the office.
Private-Pay Services
Clients choosing not to use insurance, or receiving services that are not billed to insurance, are responsible for the private-pay rate in effect at the time of service.
Insurance
Insurance eligibility and benefits may be verified before treatment. Verification of eligibility or benefits is not a guarantee of coverage or payment.
Coverage and payment are determined by the terms of the client’s insurance plan, applicable authorization or referral requirements, the insurer’s processing of the claim, and the provider’s agreement with the insurance plan.
Clients are responsible for providing accurate and current insurance information and for notifying The Massage Therapist of changes to their insurance coverage.
Patient Responsibility
Clients are responsible for applicable copayments, coinsurance, deductibles, and non-covered services as permitted by their insurance plan and the provider’s agreement with that plan.
If additional information, a referral, authorization, or other documentation is required by the insurer, treatment billed to insurance may be delayed until those requirements are satisfied.
Non-Covered Services
Some services may not be covered by a client’s insurance plan. When a service is known to be non-covered, the client will be informed of their financial responsibility when reasonably possible before treatment.
Claims & Explanations of Benefits
When The Massage Therapist submits a claim on a client’s behalf, payment remains subject to the insurer’s coverage determination and the terms of the applicable provider agreement.
Questions About Charges or Coverage
Questions about a charge from The Massage Therapist may be directed to the practice. Questions about plan benefits, deductibles, coverage limitations, authorizations, or an insurer’s claim determination may also need to be directed to the insurance company.
